A Diver Medic Technician (DMT) is the first line of medical response on a dive site. They’re trained to treat diving injuries and emergencies before a doctor can tend to the patient. This guide breaks down what a DMT does, who might train for the role, and whether it's the right next step for you.
What is a Diver Medic Technician?
Think of a certified DMT as like a diving paramedic. They act as the eyes, hands, and ears of a physician who may be hundreds or thousands of miles away.
A DMT is trained to recognize, stabilize, and treat patients who’ve sustained a diving injury or who might be suffering from decompression sickness. They deliver physician-directed care until the patient can reach comprehensive care from a doctor.
The role exists because dive teams work in remote locations where you typically don't have immediate access to a doctor or advanced medical care and when you need to treat a casualty. That's why having a qualified DMT in the team is often a requirement for the commercial diving industry, where work from ships or platforms at sea is commonplace.
What does a Diver Medic Technician actually do?
A DMT is trained to respond to the full spectrum of potential medical emergencies a dive team could face. Acting as the hands of the doctor, you have the knowledge and skills to administer immediate treatment and communicate with a physician to care for patients until a skilled doctor can take over.
A typical day in the life of a DMT is no different to that of any diver, supervisor, or support staff member. You’re not hired as DMT, but as a working professional who happens to be a capable first responder.
Treating a diver is different from treating the average casualty on the street. Diving is often conducted in remote locations increasing the technical demands placed on first aiders. The types of injuries can be different too, especially when it comes to decompression related incidents. DMTs can provide advanced first aid to a diving casualty, including therapeutic recompression, wound care, and advanced airway management. Skills such as IV cannular insertion, suturing, and catheterisation are also practiced as part of a DMT’s training.
Where Diver Medic Technicians work
Diver medics work on dive sites that are medically isolated and where outside help wouldn’t arrive in time. This spans a wider range of settings than commercial diving alone. Here's where the need for a DMT shows up most:
- Offshore and inshore commercial diving. At least one certified DMT is desirable for commercial diving operations. For offshore work, under the UK HSE's Approved Code of Practice for offshore diving for example, at least one person in the dive team must be a qualified DMT. Some projects need more than one DMT, with the diving contractor's risk assessment setting the actual figure.
- Saturation diving. Closed bell work adds a layer of risk and complexity that surface-supplied diving doesn't have, so contractors typically staff at least one DMT on every closed bell run as standard practice.
- Marine science and research. Research diving programs and institutions running remote fieldwork can require trained DMTs for the same reason commercial operations do: help is often a long way away.
- Media, expedition and advanced recreational diving. Underwater film crews and expedition teams working in remote locations may bring a DMT along for the same isolation risk, even though the diving itself isn't strictly commercial in nature.
- Military, navy and police diving. These teams operate in high-risk and remote environments where a skilled DMT is required to respond to complex emergencies.
- Hyperbaric medicine practitioners. Doctors, nurses, or hyperbaric technicians working in hyperbaric facilities or offshore medical support roles can become DMTs to build their understanding of dive medicine and diving operations.
How to become a Diver Medic Technician
Becoming a DMT comes down to four steps: meeting the prerequisites of your chosen course and provider, completing the approved training course, passing the certification exam, and recertifying through regular refreshers.
What you need before you start
Prerequisites can vary by provider and geography. For an International Marine Contractors Association (IMCA) certification, which is required to work in commercial diving as a DMT, attendees should hold an IMCA recognized diver training certificate, a diving supervisor certificate, or a valid IMCA/AODC Life Support Technician certificate.
All other students can receive a certificate issued by the training provider (for applications outside of commercial diving).
What the training covers
DMTs are trained in two connected skill sets: general emergency and clinical medicine, and medicine specific to hyperbaric environments:
Emergency and clinical skills
- Trauma management, including suturing.
- Advanced airway management, including intubation and chest drain insertion.
- Conducting neurological assessment.
- IV cannulation and IM techniques.
Hyperbaric medicine
- Applying diving-specific medical knowledge to diagnose and treat decompression illness.
- Checks and calibration of recompression chamber equipment.
Training to be a DMT involves taking a practical course backed by theory learned in the classroom. You're assessed not only on your knowledge, but by how you are as a medic, how you react, and how you interact with people.
Over the course you practise a range of medical procedures, from putting in an IV cannula, to sewing up a pig's trotter to learn correct suturing technique.
By the end of the course, you have an advanced knowledge of dive medicine, you’re able to conduct a medical assessment of a diver, and you can respond to a range of medical emergencies appropriately.
The course culminates in a mass casualty scenario where, as part of a team, you must deal with those casualties, identifying what's wrong with each of them and decide how you're going to treat them.
Recertification and staying current
Passing the exam isn't the finish line. An IMCA DMT certificate is valid for two years. After that it lapses unless you recertify.
Refresher courses to recertify keep practical clinical skills sharp. They’re necessary because a DMT's most safety-critical skills are exactly the kind of procedures you might go long stretches without performing, because diving emergencies are rare by design.
The renewal window runs from three months before your expiry date to eight weeks after it.
Is becoming a DMT a good career move?
Gaining a DMT certificate that adheres to IMCA standards means you gain an internationally recognized qualification, making it directly applicable to the commercial diving world, including desirable big-ticket offshore jobs.
Saturation and offshore teams must carry DMTs, so certified divers often get picked first and potentially command higher rates.
As a member of a dive team with a DMT qualification, you’ll be more employable to most diving contractors. Many contractors prefer to have up to two members of their team qualified as DMTs, especially for closed bell dives.
It’s worth mentioning that for the UK HSE Closed Bell Diver certification delivered by DEEP, being a DMT is a prerequisite.
